On August 6, 2014, the Office of Pharmacy Affairs (OPA) will begin the annual recertification process for all 340B participating hospitals.
Entities have until September 10 to complete recertification. Failure to do so in the given time frame will result in removal from the program.
On August 1, HRSA will send reminders out about the upcoming recertification, directed to the Authorizing Official and Primary contact (as it is listed for the Parent Site).
It is important to verify that your Authorizing Official’s e-mail address is up to date for all sites (Parent and Child) in the OPA database http://www.hrsa.gov/opa/. Recertification is directly contingent upon the Authorizing Official’s receipt of the Username and Password from HRSA on August 6th to begin the process.
For clarification purposes:
340B recertification and change request forms must be submitted by an “Authorizing Official,” which the OPA defines as “someone who can bind the organization to a contract, such as the CEO, CFO, COO, Executive Director, President or Vice President of the entity.”
